is hard to interpret. By "cardiac risk" they really mean stuff like high blood pressure, obesity, high cholesterol, and diabetes, not actual events like heart attacks and strokes. With most of the subjects being in their 20s, you wouldn't expect to see more than very rare heart attacks and strokes. But for people who are going to get these hormones for many years you might well see these serious life-threatening events occurring more frequently and earlier in life than you would otherwise expect. The extra risk of these so-called "metabolic" factors in trans people getting hormones will mean many more people will need to take drugs for these conditions (intended to reduce the chance of getting heart attacks) for many years, in addition to their hormones!

So the study raises important red flags about serious health troubles down the road, but let's not imagine that 25-year-old trans people are dropping dead from heart attacks on account of their hormones. Edit: The researchers deserve a big shout-out for doing a controlled study, too.
